Parents or guardians complete a short information sheet which is securely stored in the Clerk's office and only used when police and family report that a child is missing.  Parents receive an identification card the size of a credit card on site, complete with their child's name, digital photograph and fingerprint. The card also contains a telephone number to call after reporting the missing child to police.  The City Clerk office then produces a document containing the child's picture and information to help locate the child.

City Clerk office locations are open 8 a.m. - 5 p.m. Monday through Friday.  Other locations include City Hall, Room 107, at 121 N. LaSalle St. and 5674 S. Archer Ave.
