Emergency Identification Bracelet
The Emergency Identification Bracelet is a spandex bracelet that opens to reveal the Chicago Police Department's 24 hour telephone number of the citizien's district of residence and a 4-digit personal identification number.
This I.D. number reveals important information abou the registered senior (60+) or disabled citizen and can only be accessed by the Chicago Police Department. These bracelets have saved lives and valuable time in emergency situations, especially when the wearer cannot communicate effectively or becomes disoriented.

Department on Aging
The Chicago Department on Aging has many programs and services such as Meals on Wheels, Benefits Eligibility Checklist Program, Information and Assistance, Case Management, Chicago Fitness Plus, Golden Diners Club and Regional Senior Centers throughout the city.

VISIT THE PORTAGE PARK SATELLITE SENIOR CENTER
The Portage Park Satellite Senior Center, 4100 N. Long Ave., includes Department of Aging programs, such as fitness, wellness and computer training with Internet access. Hot lunches are now available at 11:30 am. The facility has a snack bar, a library with computers, a fitness room with treadmills and weights, and a multipurpose room with a television. Hours of operation are 8:30am to 4:30 pm Monday thru Friday. 312-744-9022
QUAILIFIED HANDYMEN & CONTRACTORS
Senior Link Alliance, located at the Portage Park Senior Satellite Center, has a list of over 60 handymen and contractors who have been prescreened and qualified to help seniors. For referrals, please call Wayne at 312-744-9070.

If you are eligible for Medicare Part D, Illinois Cares RX will help pay the costs that Medicare does not.
